CAD2Shape correctly handles translation of 'holes' or 'islands' for polygon shapefiles. It does a global resolution of all polygons in the CAD drawing to give a correct result even when polygons in the drawing are not carefully defined.

CAD file contains closed polylines inside other closed polylines. i.e. 'islands' or 'holes'.
The software does a global search for polygons inside polygons.

Resultant polygon shapefiles contain the holes.

(However the software also provides the option to not resolve holes if that result is desired.)
